Desugaring of CONSTANT_Dynamic to support JaCoCo

This desugaring supports the use of CONSTANT_Dynamic found
in JaCoCo version 0.8.4+ when instrumenting class files
with version 55 or higher.

Initial implementation use <clinit> for synchronizing constant
creation.
